Once we have a brief conversation or meeting to determine if we will be a good fit to work on your goals, I will need to collect the information listed below from you (and your partner if you are seeking couples counseling). Providing this information and completing the documents in my HIPAA-Secure Online Client Portal allows you to spend the first meeting addressing what’s important to you– instead of completing paperwork for the first 20 minutes.
Needed info for from you (or BOTH partners if you are seeking couples counseling):
- Name(s) as they appear on the insurance card
- Picture/copy of the insurance card -both front and back, or policy/subscriber numbers
- Date(s) of birth
- Cell number(s)
- Home email address(es) –where I will email you a link and password to my Secure Client Portal to complete the documents
Once I have this information from both of you, my medical records system will email you each separately a link to the online portal to complete all documentation and a password specific to you individually. It takes about 20 minutes to complete that process.
So I have plenty of time to review them, I do ask that you complete all of the client portal forms a few days before your first full appointment. I also ask you to consider signing a Client Consent for Release of Protected Health Information to your medical providers and/or psychiatrist so I can keep them informed of your progress once we start treatment.
During your initial appointment we will discuss in more depth what’s concerning you now, we will talk about anything significant from the forms you completed, and we might even be able to collaborate on a plan to help you feel better. However, sometimes it can take a few sessions to determine a plan together.
